It is said "There are a total of 5 distinct levels of identity". However, only 4 are listed.

My personal experience would make me agree that the highest level is the 4th as described: "Unity between the self and all known "external" systems". Thus, just correct this "announce".

Although, is it because a level of experience was not yet described that it doesn't exist? By extrapolating, I can imagine what could be a 5th level: "Unity between the self and all "external" systems, known and unknown" (the "unknown" being added to the 4th level). And probably, nothing can be added.

The unity with the unknown could also be a corollary of level 4, and included in the 4th level as an act of humility in its description: if I become one with the Universe, I, with my usual ego and senses, yet do not know everything about the universe. So level 4 actually implies a sort of unity with "my unknown".

But it could be more than that. "Unknown" in the 5th level could refer to the sense of the existence of unknowable dimensions for instance. Or to something greater than the Universe, in which our universe is immersed in some way...
